Today, we provide support and residential services to 300 clients living in nine group homes and apartments in Marin and Sonoma Counties.

At Lifehouse, we have developed a very progressive self-help skills training model. Our goal is to help each person become more independent and be productive members of the community.

The following is a broad overview of our services:

Assessments - Lifehouse offers skills assessments in all areas of daily living, using the TRACT assessment tool. A counselor meets with the individual either in their own home or one of our homes on several occasions for a total of 15-25 hours to assess the areas of need. This can be helpful in developing a specific service plan tailored to the unique needs of the individual.

Family living support - Lifehouse provides in-home training for clients who either want a head start on independent living skills, are waiting for a group home opening, or do not want to live in a group home. This option gives families a chance to get comfortable with the idea of their children becoming more independent. Services are for a maximum of one year if funded by the Regional Center.

Community living - Counseling and support to individuals living in their own apartments.
Supported living - Daily training by counselors to those with greater needs who live in Lifehouse facilities. On-site counselors are on call 24 hours daily.
Senior programs - Apartment living with daily assistance and nursing support that allows older adults to continue living independently. On-site counselors are on call 24 hours daily.
Independent living skills - Four group homes with overnight staff offer training for independent living. Most clients can them move to their own apartments.
Intermediate care facilities - Four group homes providing comprehensive support to individuals with severe cognitive and physical disabilities. Residents have the opportunity to live as independently as possible with outings in the community, communication training, physical therapy and nursing care.
Additional services - recreational programs promotion of housing opportunities, client advocacy, community information and referrals to other agencies.
